The medical examiner says 58-year-old Jeffrey Chapman of Reno died on scene. 

The ME says Chapman entered the ER showing a weapon and then was shot multiple times by police when he lunged at officers.

The FBI is now leading the investigation.

There is no active threat to the public.

VA Sierra Nevada Health Care System remains open and operational, and we encourage Veterans to report to their scheduled appointments as normal.
