The Carson City Health and Human Services (CCHHS) is partnering with the Carson City School District to offer students annual seasonal flu vaccinations.

Licensed nurses will be at each school to administer the vaccines and it will only be available as a shot. The nasal mist vaccine will not be available, based on recommendations from the Centers of Disease Control and Prevention's Advisory Committee on Immunization Practices.

The shot will protect against four flu viruses.

In order for students to receive the vaccine, they must:

Complete and sign the consent form here: http://www.carsoncityschools.com/UserFiles/Servers/Server_801527/File/Dan Davis/2018 Flyer, Letter, Consent, Info.pdf

Send the completed form back to the student's school or teacher at least one week before your school's scheduled clinic date.
